Magnus Health Portal

Westchester Day School uses the Magnus Health Portal for all medical records. To access your child's Magnus portal please log in to your MyWds on the top right of the page and find the "Magnus Health Portal link" in the parent resource board. 

Emergency Information:  Must be filled out for each child registered for Westchester Day School.
Medical Forms: Print the necessary form by clicking on the appropriate link below.  All forms must then be uploaded through the Magnus Portal.

Files: 

• Health/Immunization Form or we will accept the form that your physician provides.
• Prescription Medication Administration Form -if applicable
• Over-the-Counter Mediation Administration Form- if applicable
• Asthma Action Plan Form- if applicable
• Food Allergy Action Plan Form- if applicable
• Diabetes Action Plan Form- if applicable
• Seizure Action Plan Form- if applicable
